For a baby’s cot, choose a waterproof mattress protector only if it fits the exact mattress snugly and stays flat. It should not add soft padding or compromise the firm, flat sleep surface. There is no evidence here to name a tested “best” model: this is a practical guide to choosing a protector, not a product ranking.
What “cot pad” means—and what this guide covers
“Cot pad” can mean either a waterproof mattress protector or a soft topper. For an infant’s sleep space, those are not interchangeable: this guide covers fitted waterproof protectors, not cushioned toppers. A thicker or softer pad is not a safer or more sleep-promoting choice.
The advice below focuses on UK cot and cot-bed use. Cot dimensions and terminology vary by market, so a product described as a US “crib mattress pad” should not be assumed to fit a UK cot.
Start with the sleep surface, not extra padding
UK NHS guidance says a cot mattress should be firm, flat, waterproof and a snug fit, with a waterproof cover. See NHS: What you’ll need for your baby and NHS: Baby safer sleep advice. The Lullaby Trust likewise advises checking firmness, flatness, waterproofing and fit: Baby sleeping products.

US guidance is phrased differently. Safe to Sleep, from NIH/NICHD, advises a firm, flat, level surface and says to remove everything from the infant sleep area except a fitted sheet covering the mattress: Safe to Sleep: Reduce the risk. The American Academy of Pediatrics recommends a firm, flat, noninclined surface covered by a fitted sheet, without other bedding or soft objects, in its 2022 policy statement. Hong Kong’s Family Health Service also advises a firm, well-fitted mattress, no gaps and no soft objects or loose bedding apart from a fitted sheet: Safe Sleep Sweet Dreams. Follow the current safe-sleep guidance where you live rather than treating one jurisdiction’s wording as universal.
How to choose a cot mattress protector
- Check exact compatibility. Measure the cot mattress and check the protector’s stated dimensions and intended use. Confirm it suits both your cot and mattress; “cot,” “cot-bed” and “crib” sizes are not universal.
- Look for a snug, flat fit. The protector should remain securely fitted without bunching, shifting or leaving gaps around the mattress. Do not use it if it changes the fit of the mattress in the cot.
- Choose waterproof protection without treating padding as a benefit. In UK guidance, the mattress should be waterproof and protected by a waterproof cover. A soft or thick layer is not a substitute for a firm, flat mattress.
- Read washing and drying instructions. Follow the manufacturer’s care directions so the protector can be cleaned and refitted correctly. Do not put it back on if it is damaged or no longer fits as intended.
- Check age and use directions. Follow the product maker’s instructions, and do not infer suitability for every cot from a general product description.
Why fit and compatibility matter
A protector is not a way to correct a poorly fitting, soft or uneven mattress. The US Consumer Product Safety Commission warns that aftermarket mattresses used in incompatible sleep products, or as toppers or supplemental padding, can pose a significant hazard. Check the exact product compatibility and dimensions: CPSC: Crib Mattresses FAQs. If the protector makes the surface uneven, loose or less snug, do not use it.

A product example is not a “best” verdict
Sealy describes its Cozy Dreams Waterproof Fitted Crib Mattress Pad, model ED021, as a waterproof fitted pad with a skirt that wraps under crib and toddler mattresses: Sealy Baby product page. That description does not establish compatibility with every cot or mattress, nor does it amount to an independent comparison or endorsement. Check the product’s current dimensions and instructions against your own sleep space before considering it.

A mattress protector should not be presented as preventing SIDS or making a baby sleep longer. The FDA says products claiming to prevent or reduce SIDS have not been cleared or approved for that use, and warns against positioning devices: FDA recommendations for parents and caregivers. Choose a protector for fit and waterproofing, not unsubstantiated safety or sleep claims.
