The right surface depends on pressure-injury risk, ability to reposition, skin and moisture needs, comfort, mobility, body weight, bed fit, and care goals. Before choosing, clarify whether you need a static support surface, a powered alternating-pressure mattress, a low-air-loss surface, or an overlay—and involve the care team when needs are complex.
What “air mattress” means in a hospital bed
The term can describe different support surfaces. Their functions are not interchangeable, and some products combine more than one mode.
Static or reactive support surface
A static surface changes how it distributes a person’s load in response to pressure; it does not use a powered cycle to shift air between chambers. The Agency for Clinical Innovation (ACI) says a static surface may be considered when a person can be positioned without loading an existing pressure injury, does not bottom out, and does not develop red marks on supporting sides. These are selection considerations, not a diagnosis or prescription. ACI’s mattress-upgrade and selection guidance explains the distinctions.
Alternating-pressure mattress
A powered alternating-pressure mattress uses a pump to fill and deflate alternate air chambers over time, redistributing pressure during the cycle. ACI suggests considering a dynamic surface when pressure cannot be removed from an injury, a static surface bottoms out, healing is not evident, or red marks develop. A powered surface should still be assessed for fit, stability, and whether it can redistribute pressure without bottoming out for that person.

Low-air-loss surface
Low-air-loss features address the skin’s microclimate, including moisture management, as well as pressure support. A product may combine low-air-loss with static and alternating-pressure modes. For example, Stryker describes its IsoAir as a deep-cell low-air-loss support surface with static and alternating low-pressure modes; that is the manufacturer’s description of one product, not independent evidence that it will improve outcomes for a particular person. Stryker’s IsoAir product page also reports case-study outcomes, which should not be treated as estimates of what another user or facility will experience.
Overlay versus replacement mattress
An overlay sits on top of the existing mattress; a replacement surface takes the place of the standard mattress. Check the specific product instructions to confirm which configuration it requires, whether the bed is compatible, and whether a foam mattress surround is needed. A product described as an “air mattress overlay for hospital bed” is not automatically compatible with every base mattress or articulating bed.

What the evidence says about choosing a surface
Evidence does not establish one support-surface type as best for everyone. The American College of Physicians (ACP) guideline summary recommends an advanced static mattress or overlay rather than a standard hospital mattress for patients at increased risk of bedsores. For that population, ACP says evidence did not show a clear preventive benefit for alternating-air mattresses or overlays; it also notes uncertainty and limited evidence for advanced surfaces in treatment. This is a specific guideline’s framing, not a claim that powered air is never appropriate. ACP’s guideline summary describes its recommendations.
ACI’s practical selection guidance addresses individual circumstances in which a dynamic surface may be considered, such as when a static surface bottoms out or a person cannot be positioned without pressure on an existing injury. Anthem’s medical policy likewise summarizes pressure-mattress evidence while noting that comparisons among mattress types remain inconclusive and that many studies have limitations. Coverage policy is payer-specific, not a universal clinical recommendation. Anthem’s pressure-reducing support surfaces policy provides that evidence context.

How to compare hospital-bed air mattresses
Compare the surface against the user’s needs and the bed’s setup—not just a product’s “comfort” label. Ask the clinician, equipment supplier, or occupational therapist to help resolve clinical or fit questions.

ACI notes that active surfaces can be less stable than more stable static mattresses, even when they may offer a higher level of pressure relief. It also notes that bottoming out can differ between mattresses within the same category. These trade-offs make assessment of the person and bed essential.

Safety: a mattress is only one part of care
A support surface does not eliminate pressure-injury risk or the need for repositioning. ACI states: “A support surface does not eliminate the risk of new pressure injuries or the need for repositioning.” Follow the care plan for repositioning and skin checks, and raise new or worsening skin changes with the care team.
Do not add pads, slide sheets, slings, or other materials unless the manufacturer’s instructions allow them. ACI warns: “Do not place anything on top of a pressure redistribution mattress other than what is recommended by the manufacturer as any material will negate the pressure relieving properties of the mattress.” Confirm bed and mattress dimensions, and assess entrapment, fall, transfer, and bed-height risks as part of setup.

For children, do not apply adult product advice without specialist assessment. ACI recommends occupational-therapist assessment and notes that alternating-air mattresses may be unsuitable in some pediatric cases: large cells can leave gaps, low body weight may not trigger sensors correctly, and some models have a permanently inflated head section.
