Standing desks can reduce the time office workers spend sitting, but that does not prove they prevent disease or make users healthier overall. Nor does the evidence show that standing for long stretches is a better substitute. The practical aim is to vary posture and add movement—not to trade one static position for another.
What standing desks have been shown to do
The clearest supported benefit is behavioral: sit-stand desks can help people sit less at work. A 2025 systematic review by Silva and colleagues reported average reductions in full-day sedentary behavior of 68.7 minutes per day at three months, 77.7 minutes at six months, and 62.1 minutes at 12 months, compared with control groups. For sitting during working hours, the review reported reductions of 73.0 minutes per day at nine weeks, 88.0 minutes at three months, 80.8 minutes at six months, and 48.0 minutes at 12 months. These are review-level averages, not guaranteed results for an individual worker. Read the 2025 systematic review.
Those reductions matter as a change in behavior, but they do not by themselves establish a health outcome. A 2018 Cochrane review described low-quality evidence that sit-stand desks reduce workplace sitting by an average of 84 to 116 minutes per day in the short and medium term; that review found no evidence on longer follow-up periods. The evidence supports less sitting, not a promise of disease prevention. See Cochrane’s 2018 review.
Why less sitting is not the same as better health
Standing still is not the same as moving. A desk can make it easier to change position, but the fact that someone stands more does not establish that they are getting the benefits of light activity or exercise. The health effects of sit-stand desks remain less certain than their effect on sitting time.

A 2019 scoping review indexed by NIOSH examined 53 articles, with study samples ranging from 6 to 231 people and follow-up periods from one day to one year. Across the results reviewed, statistically significant findings appeared in 61% of behavioral results, 37% of physiological results, 31% of psychological results, 43% of discomfort results, 18% of posture results, and 7% of work-performance results. Those percentages describe results within research domains—not the share of workers who benefited. The review concluded that desks can change behavior, while effects on health outcomes were more modest, and called for more evidence on long-term effects and appropriate dose and setup. Can standing for long periods cause problems?
The reviewed evidence does not establish that standing desks are inherently harmful, nor does it prove that prolonged standing is a safe or beneficial replacement for prolonged sitting. It also does not establish a single ideal sit-stand schedule. The sensible caution is to avoid treating standing as a target to maximize: changing position and moving around are different from remaining upright and still.

On musculoskeletal symptoms specifically, a 2019 Cochrane review of workplace interventions to increase standing or walking found that the limited available evidence did not show reductions in symptoms at short-, medium-, or long-term follow-up. The authors rated the evidence low or very low quality, so this is not proof that no worker can feel better; it means the studies did not establish a reliable symptom benefit. What desk setup makes sense?
Choose a setup that lets you work comfortably and change posture without disrupting the task. The evidence does not establish one desk type as universally best. A 2015 systematic review found few physiological changes for standing desks, compared with selected effects from treadmill desks, while identifying evidence gaps and little impact on work performance across the desk types reviewed. - Fixed standing desk: Provides a standing work surface but does not, by itself, support switching between seated and standing work.
- Height-adjustable sit-stand desk: Allows a worker to alternate work positions. Its best-supported outcome is reduced sitting time, not a guaranteed health improvement.
- Treadmill workstation: Adds walking to desk work, but its suitability depends on the task and user; the reviewed evidence does not establish it as the best option overall.
For desk-based occupations, a 2015 expert statement recommended initially building toward two hours per workday of standing and light activity, progressing toward four hours. That is expert guidance—not a proven optimal dose, a universal prescription, or a reason to stand continuously. See the 2015 expert statement.
How to use a sit-stand desk without treating it as a cure
- Use the desk to vary posture. Alternate between sitting and standing in a way that fits your comfort and work. The evidence does not establish one schedule that is right for everyone.
- Make room for movement. Take opportunities to walk or move lightly rather than counting every minute upright as activity.
- Match the position to the task. Use the posture that lets you work comfortably and effectively; a desk is a workstation option, not a medical treatment.
- Respond to discomfort. Change position if a posture becomes uncomfortable. If symptoms persist or concern you, seek individualized advice from a qualified health professional.
What remains uncertain
The 2025 review found sitting-time reductions at the follow-up periods it analyzed, but said long-term effectiveness and effects beyond the workplace remain uncertain. Across reviews, evidence is mixed or limited for physiological, psychological, discomfort, and work-performance outcomes. The available findings therefore support a modest conclusion: these desks can help change how much people sit, but they do not guarantee better health, and the evidence does not define a universally optimal amount of standing.
