Inspect an RCC (reinforced cement concrete) box culvert by checking the barrel, joints, inlet and outlet structures, and surrounding channel for cracking, water passage, soil loss, movement, and erosion. Record what you see and how it changes; visible warning signs can help determine when to call a qualified inspector, but a visual walk-through cannot certify the culvert as safe.
What to inspect on a concrete box culvert
Include the full structure and its approaches in the walk-through, not just the visible barrel opening. Where safely accessible, look at the inlet and outlet, barrel walls and roof or invert surfaces, joints, headwalls, integral wingwalls, apron, downstream channel, and visible scour protection. FHWA’s culvert condition framework considers alignment, settlement, joints, structural condition, and scour as part of an overall evaluation (FHWA Recording and Coding Guide).
- Concrete: Note cracks and their pattern, spalls, exposed reinforcing steel, or areas that appear deformed.
- Water and joints: Look for seepage, active leakage, infiltration or exfiltration, open joints, and signs that water is carrying soil.
- Ground and support: Look for settlement, misalignment, undermining, exposed foundations, voids, and erosion around the structure.
- Appurtenances and channel: Check headwalls, wingwalls, apron, and erosion protection for cracking, displacement, deterioration, or loss of support.
How to record findings
Make observations comparable over time. For each issue, record its location and extent, whether it is wet or actively passing water, whether soil appears to be moving through it, and whether it has changed since earlier photographs. Also note exposed reinforcement, displacement, settlement, or other visible movement. Take photographs from repeatable positions where safe; a crack-width gauge card may help document measurements, but it is a recording aid, not a safety assessment.
Do not enter a culvert to complete a visual check unless the responsible owner’s safety procedures and qualified personnel authorize and manage the work. Culvert interiors may present confined-space, water, traffic, and unstable-ground hazards. Inspection access and intervals should follow the owner’s requirements and applicable jurisdictional rules.

Which cracks, leaks, and erosion signs warrant escalation?
FHWA’s Federal Lands Highway assessment procedure identifies several poor-condition indicators for concrete boxes and arches. These are criteria from that specific procedure, not universal limits for every owner or jurisdiction. The procedure links a poor condition to repair actions; the appropriate response for a particular culvert still depends on an engineering assessment and the owner’s procedures (FHWA HDS-5, Third Edition).
- Cracks wider than 1/4 inch (6 mm) when accompanied by significant infiltration or exfiltration and voids.
- Cracking of any size covering more than 50 percent of the culvert surface area.
- Exposed reinforcing steel, or open joints with significant soil and/or water passage and visible voids.
- Perceptible cross-section deformation.
- Substantial or widespread cracking, active seepage with soil migration, visible voids, settlement, or misalignment. These observations merit prompt assessment even when a specific threshold above has not been established for your owner or jurisdiction.
Check the supporting structures and erosion protection as carefully as the barrel. Under the same FHWA procedure, a headwall, wingwall, or apron is rated poor when more than half its surface is cracked and spalled, when reinforcement is exposed, or when its foundation is exposed or undermined. Scour protection such as riprap is rated poor if displacement, undermining, or deterioration significantly affects its performance or the culvert itself.

How to prioritize maintenance and repair
Routine maintenance can preserve flow and help reveal changes at the inlet and outlet. Structural defects and active soil loss need assessment before cosmetic patching: covering a visible crack without understanding the water path or soil movement may leave the underlying problem unaddressed.
When an inspector evaluates possible repairs, the decision depends on the defect and site conditions. Useful questions include whether the issue is structural or hydraulic, localized or widespread, dry or actively passing water, associated with soil loss or voids, confined to the barrel or also present in the headwalls, wingwalls, or apron, and stable or progressing. Repair planning also needs to account for flow capacity, access, constructability, durability, foundation support, and the water and soil path. These are decision factors, not a universal FHWA ranking of repair methods.

A general inspection guide cannot prescribe one sealant, grout, lining, riprap size, or repair sequence for every RCC culvert. Hydraulic loading, foundation conditions, reinforcement, access, and jurisdictional standards affect what is suitable. FHWA’s hydraulics catalogue lists the Federal Lands Highway Culvert Assessment and Decision-Making Procedures Manual (2010) and HDS-5 (2012); FHWA identifies its 1986 Culvert Inspection Manual as archived and of historical value. These publications do not replace an assessment under the asset owner’s current requirements.
